ID Verification and Insurance Card Form

Step 1: Take a picture of the front and back of your driver’s license and your insurance card for a total of four (4) pictures.

Submit from your Iphone:
 Click on “Choose File” below
 Choose picture from Photo Library
 Choose picture size (please reduce to small)
 Choose Back to All Photos / Recents
 Choose Done
 Click “Send” button to finish

Submit from your Computer:
 Send pictures to your email (please reduce to small)
 Open your email and select pictures
 Name and Save each picture to your Desktop
 Go back to BCC website to the “ID Verification and Insurance Card” section
 For each picture, click on “Choose File” choose the corresponding “Browse” link
and upload your Pictures.
 Click on “Choose File” for each picture and upload your pictures
 Click “Send” button to finish

Please attach pictures of your Driver’s License.
